Indiana Medicaid HCBS programs
Health & Wellness Waiver
For eligible individuals age 59 and younger with disabilities who meet Medicaid, HCBS, and nursing-facility level-of-care requirements. There is no minimum age.
Indiana PathWays for Aging
For eligible individuals age 60 and older with full Medicaid coverage through PathWays; nursing-facility level of care is required for the waiver itself.
Important: Medicaid/waiver eligibility and service authorization are determined by Indiana FSSA/OMPP processes. Age Well Indiana does not determine eligibility.
