What we do

Two services, delivered under your approved plan.

Age Well Indiana is approved for two Indiana HCBS waiver services. What we deliver, and how much of it, is set by the member's authorized service plan.

Attendant Care

Hands-on, supportive care for someone who is medically stable — help with the everyday things that make staying at home possible.

Help with personal care and daily routines, as authorized
Reminders to take medication
Support at home and in approved community settings

This is non-skilled care. It does not replace a nurse or other licensed health professional. Exactly what is covered, and the limits on it, are set by Indiana's current waiver rules and the member's approved service plan.

Home and Community Assistance

Practical help with running a household and taking part in community life, so a person can keep living in their own place.

Which Indiana waiver covers this?

On July 1, 2024, Indiana's former Aged & Disabled Waiver was divided into two waivers — one for people 59 and younger, one for people 60 and older.

About “nursing facility level of care”: this is a test the State applies to the individual, not to the provider. A person has to be assessed as needing that level of care so the waiver can pay for them to be cared for at home instead. Age Well Indiana is not a nursing facility.

Age 59 and younger

Health & Wellness Waiver

A 1915(c) HCBS waiver providing an alternative to nursing facility admission. There is no minimum age — it serves eligible children and adults through age 59.

Full Medicaid in a compatible aid category, with income no greater than 300% of the maximum SSI amount
Aged, blind, or otherwise disabled, and assessed by the State as meeting its level-of-care standard
Living in, or moving into, an HCBS-compliant (non-institutional) setting

Age 60 and older

Indiana PathWays for Aging

Indiana's managed long-term services and supports program for Medicaid members 60 and older in an aged, blind or disabled category. Waiver services run through the member's assigned health plan.

Age 60 or older, with full Medicaid coverage under the program
The State's level-of-care assessment applies to the waiver services, not to PathWays membership itself
The waiver benefit plan and service plan must be approved before covered services begin

Eligibility

Do I qualify?

Age Well Indiana does not decide this. The State does. Here is who actually makes each decision, and how to reach them.

Medicaid

You need the applicable Indiana Medicaid coverage and must meet the program's eligibility rules.

Level of care

Indiana's Level of Care Assessment Representative (LCAR) assesses whether your needs meet the State's standard for waiver services.

Service plan

Your approved plan has to authorize the specific services and the number of units.

Choose a provider

Once services are authorized, you may choose an available approved provider — such as Age Well Indiana.
